Top client in looking for an LPN Case Manager!

MAJOR DUTIES: This position works under the supervision of the Director of Nursing and works closely with Director of Withdrawal Management, Admissions Supervisor and Director(s) of Residential Units. This position is responsible for coordinating clinically and complex patient's care across a continuum, ensuring and facilitating the admissions and intake of patients admitting to withdrawal management and residential service. This position provides clinical services to agency patients.

Qualifications:

In fulfilling these duties, the incumbent performs the following duties independently:

  • Work closely with interdisciplinary care team and technology to identify high-risk,
  • complex patient cases; manage care, including referral management.
  • Develop a cohesive and strong team-oriented relationship with physicians, nurses and
  • other healthcare professionals and drive consistent coordination of care.
  • Assist with admissions to inpatient units and transitions for patients discharged from
  • one program to another.
  • Document the plan of care in appropriate EHR systems and enter data per specified
  • Quality assurance of clinical documentation
  • Review medical complexity for inpatient units and determine appropriateness for
  • admission
  • Ensure adherence to State, Federal, Local and company policies and procedures.
